Ingredients

  • Sichuan pepper (huajiao) 30–50 g in published perianal formulas (see body for vulvar reduction)
  • Salt 10–30 g in sources (3–5 g or omit for vulva)
  • Rice vinegar ~1/3 cup optional (perianal formula with weeping erosion)

Principle

Pepper is acrid and warming—disperses and relieves itch; salt softens. Sources target **hemorrhoids, perianal itch, tinea pedis**—thicker skin—not a dedicated vulvar formula. Vulvar mucosa is thin and easily irritated (syndrome differentiation required).

Effects & uses

Documented for perianal/anorectal sitz baths or compress washes. **Vulvar use is optional, low-concentration, post-diagnosis only**—prefer sophora, cnidium, phellodendron, kochia, purslane washes for vulva.

Method

  1. **Version 1 (Zhejiang Provincial Zhongshan Hospital clinician, perianal)**: 30–40 peppercorns decocted, add 10 g salt, sitz bath when warm; optional vinegar.
  2. **Version 2 (Prof. Lu Qi anorectal formula)**: 50 g pepper, 30 g salt, soak in 6–8 cups warm water 2 h, boil 5–10 min, compress wash 3× daily.
  3. **Version 3 (Postoperative anal canal care trial)**: Pepper–salt fumigation wash compared favorably with conventional PP sitz bath (see original paper).

How to use

  • Published use is **perianal/feet**. **If ever used on vulva**: diagnose first; ~10 peppercorns, clear decoction only, 3–5 g salt or none, ~37°C, no steaming, external vulva only; stop if burning or redness worsens.

Caution

Do not use without diagnosis. High pepper/salt may burn thin vulvar skin and mask infection. Avoid during menses, broken skin, or pregnancy when uncertain. Not a substitute for exam or treatment.
